CFC Member Feedback form 2006/7 Season.

We need our members to give us something to work with to plan the upcoming season, so here's a little form that we'd like you fill in and submit back to us before 2nd September 2006.

At the end of the form is also a current address and contact details form, please fill this in so we can have you at our fingertips when we need you.

Playing status (assuming you're selected of course!)

I'll play in as many games as possible

I'll play in up to half the games

I'll play about 1 in 4

I'd like to train and I'll fill in the odd game if really needed

I'd like to train only, I never want to pull on a shirt and feel the glory

I'm in it for beer and laughter only (Social member)

Commitment preference

I'd like to know if I'm playing up to a month in advance

I'd like to know if I'm playing no later than at Training on a Tuesday

I'm ok with being picked on a Thursday and told by e-mail

I really don't care - just tell me and I'll be there.
